Voluntary Life
Voluntary Life is available to purchase additional coverage for yourself, and/or for your spouse and children. Employees can choose amounts in $10,000 increments up to $500,000 or five times your annual salary, whichever is less. You may purchase $5,000 increments up to 50% of your election amount or $250,000 for your spouse, and a $10,000 coverage amount for your children. The guaranteed issue amount for employees electing benefits within 30 days of hire is $200,000 for the employee, and $30,000 for a spouse. For any amount over the guaranteed issue limit, an Evidence of Insurability (EOI) is required. You must be enrolled in order to enroll your spouse or children.
During open enrollment periods, employees may increase their existing life insurance coverage up to $20,000 more without any additional paperwork needed. Spouses may increase their existing coverage up to $10,000 without any additional paperwork needed. If you are an existing employee enrolling in life insurance for the first time during open enrollment, you are guaranteed an election amount of $20,000 for you and $10,000 for your spouse. For any amount over the guaranteed issue limit, an Evidence of Insurability (EOI) is required.
Short-Term Disability
Short Term Disability (STD) protects you and your family from a loss of income if you are unable to work due to an illness, injury, or accident for a short period of time. The STD plan provides 60% of your base pay up to $1,000 per week and continues for up to 13 weeks of disability. You must use all accrual leave and be in a no-pay status before your qualifying short-term disability pay will begin.
Long-Term Disability
Long-Term Disability (LTD) is important because anyone at any age may become injured or ill for an extended period of time. LTD protects you from a loss of income in the event you are unable to work due to an illness, injury, or accident for a period of time that exceeds the short-term disability benefit.
The LTD coverage provided by City of Baytown will replace 60% of your base salary to a monthly maximum of $5,000 if you are disabled for more than 90 days and are unable to work. LTD benefits start on the 90th day of being unable to work. LTD benefits are offset with other sources of income, such as Social Security and workers’ compensation.
